Case 1: Male Abdominoplasty in the Massive Weight Loss Patient
- Many patients present with excess skin and fat in the abdomen after weight loss, such that the mass effect created by this excess tissue interferes significantly with everyday life. Therefore, even though many of these patients are not yet at their ideal body weight, surgical removal of this redundant skin and fat can significantly improve the overall quality of life. This video highlights the principles that underlie the proper and safe application of the preoperative marking pattern along with a detailed demonstration of the technical details designed to maximize the result with few complications. Details related to incision placement, flap development, abdominal wall management, tissue resection, and management of the umbilicus are presented.
Case 2: Bilateral Augmentation/Mastopexy With Full Abdominoplasty
- This case demonstrates two commonly performed procedures in the field of breast and body contouring, namely augmentation mammaplasty and full abdominoplasty. The patient presents with a significant breast asymmetry and desires an increase in volume along with a concomitant breast lift. Many surgeons consider this combination of procedures to be one of the more difficult tasks in plastic surgery. This case highlights a strategy for creating a treatment plan that uses a circumvertical skin pattern along with a subfascial implant placement to both lift the NAC and narrow the breast with increased projection. The technical steps involved in choosing asymmetrical implants to enhance symmetry are also outlined. Finally, the technical details involved in using the interlocking technique to manage the lift of the NAC are presented.
- Concomitantly, the case demonstrates a basic approach applicable for many patients seeking abdominal rejuvenation. Particular attention to the marking strategy is presented that helps ensure an adequate amount of soft tissue is removed for the best aesthetic result without creating a lateral dogear that could subsequently require revision. A strategy for management of the lax abdominal wall is also presented that is straightforward and results in consistent and long-lasting reshaping of the abdomen. Finally, a technique for umbilical repositioning and inset is demonstrated that provides optimal positioning with an aesthetic appearance. Taken together, this video demonstrates a basic technique and approach that is applicable to a wide variety of patient presentations.
